The forms listed below are for residential service only. For commercial service, please call the Customer Service Phone Center. Please allow at least three business days (excluding holidays) to complete your order.
Start Service
If you are moving to a new residence and need service established in your name, use this form (residential service only).
Stop Service
To take your current service out of your name, use this form (residential service only). If you need to establish service at a new residence, please use the Transfer Service form instead.
Transfer Service
If you currently have service with the City of High Point and are moving to an address that is also served by the City of High Point, use this form (residential service only).
Change Phone/Mailing Address
We use this information to respond to you as quickly as possible when necessary. If the phone number or mailing address on your bill is incorrect, please use this form to correct it. Please note this form is used to modify/add your contact information only.
